The patient developed symptoms, including high fever, chills, and dizziness, on January 21. The following day, he sought treatment at the public hospital, where he was hospitalized. Physical examination revealed painless eschars on his left shoulder and neck. Based on these signs and clinical treatment, scrub typhus was suspected. The patient had recent exposure to rats, though his family members remain symptom-free.
