SSM advances community-based chronic disease management

The Health Bureau is enhancing community engagement through its “Community Health Committee,” which holds monthly meetings across the city’s districts to collect input from residents on healthcare policies. A recent session, attended by over 100 participants, discussed chronic disease management and the optimisation of community health services. Officials noted that the “Healthy City” initiative has involved more than 90,000 residents and established 146 self-testing stations, while a new chronic disease programme integrates smart health platforms and AI-assisted diagnostics to improve care and monitoring.
